Office Visits
Please call for an appointment at least four weeks before you want to see the
doctor. Have all your current medical insurance information available when you
call. Our insurance specialists will help determine what you must do prior to
the office visit.

New Patients
As a new patient, you will receive a short questionnaire regarding your cardiac
history, general health and medications. This questionnaire must be fully
completed and brought with you to your first appointment.

Please arrive at least fifteen minutes prior before your appointment in order to
complete the required paperwork. We suggest you allow at least one hour for your first
appointment.
Making the Most of Your Visit
Please allow sufficient time for your appointment. If you are an existing
patient, we ask you to allow at least one hour for your visit. Feel free to call
the office just prior to your appointment time to ask if the doctor is on
schedule.
Please arrive at least fifteen minutes prior to your appointment to allow time
for completing the registration process.
To make the most of your visit, it's helpful to prepare a list of questions and
bring a list of all medications you are currently taking. This list will be
noted in your medical record.
During your appointment, be sure to review your treatment plan and medications
with your doctor. If you need additional information, educational materials are
available for you to take home.
Prescription Refills
Be sure to request medication refills at the time of your office visit. If you
need refills between office visits, please call during normal business hours.
Provide the name of your medication, the dose, frequency and your pharmacy's
telephone number. Allow up to 24 hours for the prescription be called in to your
pharmacy.
If you have not been examined in the office within the past year, we request you
have an office visit prior to medication being prescribed.
